1. The entry wound was the forehead, some tiny hole but around it is... not really scorchmarks? But definitely the gunpowder reside of, say, a flintlock going off right on someone's head. The exit wound is right above the base of the neck and is essentially just a whole mess of blood and brains and general un-fun for everyone.

2. The only wound she has is the source of her concussion - a huge bruise on the top-back of her head, pretty much right to the side of her topknot - where she visibly was just bopped on the head. And considering her spotty memory, it was one hit from behind and done.
